N2 - Mandibular molars are the most commonly affected by caries that seek endodontic therapy for their extended survival. Due to complex anatomy and morphological variations, the disinfection becomes a challenge. Because of modern advents like microscope, CBCT and various magnification aids, it helps to overcome this problem. Middle mesial canal in lower molars is one such variation that might be overlooked, difficult to locate and missed which makes the tooth leads to failure of the endodontic treatment in the long run. The present case report describes the method of detection and management of complex internal anatomy and also gives clinical tips for search of additional canals.
